Overview

CHAT provides health literacy training to adults with disabilities, supporters and family members. 

Major Activities

  • Participants with disabilities will learn about:

    • How to use person centered approaches to become a responsible health partner
    • How to develop a one page person centered health profile to be shared with medical staff
    • How to better communicate health concerns with doctors, healthcare providers, and supporters
    • How to speak up and ask specific questions about their health
    • How to prepare for medical visits and identify health priorities

Target Audience

Adults with disabilities who are interested in developing health literacy communication skills.
